Cardiologists have identified a small anatomical feature in the heart known as the left atrial appendage (LAA)—a tiny pouch off the left atrium—that’s responsible for the vast majority of stroke-causing clots in patients with atrial fibrillation (AFib). In fact, about 90% of thromboembolic strokes originate in this pouch. Surgeons sometimes remove or seal it to reduce stroke risk.
This LAA, often described as a “secret pocket” with no clear purpose, can trap blood during irregular heartbeats, leading to clot formation and increasing stroke risk.
Doctors say your heart may have a tiny pouch that plays a big role in many strokes.
This pouch is called the left atrial appendage (LAA). It’s a small, ear-shaped sac in the upper left chamber of the heart. In people with an irregular heartbeat—especially atrial fibrillation (AFib)—blood can slow down and collect in this pocket. When blood pools, it can form a clot.
If a clot escapes from the pouch, it can travel to the brain and block blood flow, causing an ischemic stroke. Research shows that in AFib patients, about 90% of stroke-causing clots start in the LAA.Why it’s called a “secret pocket”The LAA is not well known to the public because it has no essential

Another lesser-known pouch, the left atrial septal pouch, is found in about 1 in 3 people. This is a small space in the wall between the left and right atria. It can also allow blood to stagnate and clot, especially in people who’ve had strokes with no obvious cause.
